Learning Review Mary 2018

A Learning Review for 'Mary' was published in December 2018 and completed by independent reviewer Maureen Noble.

Mary was an older lady who passed away in hospital with a number of serious medical conditions and after being admitted in a very poor physical state, though there were no injuries or marks in line with physical abuse identified. Mary's son was arrested on the night she was admitted to hospital for 'allowing a vulnerable person to suffer serious physical harm' and police decided to conduct a Domestic Homicide Review ('DHR'). This was later withdrawn as there were no grounds for prosecution, however the Salford Safeguarding Adults Board had contributed to the DHR and it was felt the learning from this review was still useful for agencies in Salford.
