SAR Francis (2024)

A Discretionary SAR for 'Francis' was accepted and signed off by the SSAB on 31st May 2024 and published in June 2024. The report was written by an external author, Eliot Smith. (This report has been shared with the National SAR Library).

Francis was a white British male who was in his 50’s at the time of his death. Francis lived alone and had limited contact with his family. Francis did not have a partner, or children and his social network consisted of some close friends and neighbours. Francis received support mainly from his small social network, including help with his home and pets but when he was willing to accept the support was sporadic. Concerns about Francis’ vulnerability and risk of exploitation were raised by others in his network with concerns about the circumstances of his death.

The review identified the following learning themes:

- Exploitation
- Drug addiction
- Self-Neglect
- Importance of timeliness in decision making
- Multi Agency Risk Management
- Professional Curiosity
