SAR JW 2018

A Mandatory SAR for 'JW' was published in 2018 and completed by independent reviewer Julie Pett.

JW was a lady with severe learning disabilities and complex mobility, health and communication difficulties, who passed away in hospital at 55 years old. No abuse or neglect was suspected or identified as contributing to JW's death, though it was found that agencies could have improved how they worked together when responding to her complex needs.
